Compare the Richmond City Health Department organization, which answers only to a City Manager, but has 60% of budget provided by the state with it sister agency , the VDH, and the Corpus Christi agency.   Why might it be organized differently?  What might be a difference in strategic objectives?  Consider the advisory Boards. Are they needed?  Why?  What management role do they fill?
